The United States Central Command (Centcom) has confirmed the deaths of six American military personnel following the crash of a KC-135 refuelling aircraft in western Iraqi airspace on Thursday, 12 March. Although investigations remain ongoing, the Pentagon has asserted that the aircraft’s loss during Operation Epic Fury was not caused by enemy attack or friendly fire.

Separately, Iran has claimed that the tanker aircraft crashed as a result of a direct attack by air defence systems operated by resistance forces in Iraq.

This incident adds to the grim military toll for the United States since fighting with Iran commenced on 28 February, with total personnel deaths reaching 13. However, this figure has prompted debate following a Pentagon press conference in Washington marked by uncertainty regarding casualty data.

Defence Secretary Pete Hegseth appeared hesitant when pressed by journalists regarding exact casualty numbers and locations of deaths. “War is hell, war is chaos,” Hegseth stated before deferring his response to Chairman of the Joint Chiefs of Staff, Dan Caine.

Caine cited casualties occurring in Kuwait, Jordan, and southern regions resulting from drone strikes (one-way attack strikes). Hegseth claimed that 90 per cent of wounded personnel have returned to duty, but he refused to provide exact figures distinguishing between killed in action (KIA) and wounded in action (WIA), further fuelling public speculation.

Suspicions regarding higher casualty numbers have been reinforced by reports from Military Times. The Pentagon’s largest medical facility in Germany has reportedly suspended general patient services to focus on treating combat injuries. This followed the evacuation of 19 personnel from Saudi Arabia to Germany following an unmanned aircraft attack.

Meanwhile, the Institute for the Study of War (ISW) has offered critical analysis. Whilst the United States denies that the KC-135 crashed due to an attack, ISW notes a series of attacks by Iran-backed militias against US forces in Iraq during the same period.

The escalation in geopolitical tensions across the Middle East has intensified following drone strikes targeting US facilities in Baghdad on Tuesday, 10 March, amid the broader conflict between Iraqi and Iranian forces. Defence Secretary Hegseth has accused Iran of firing missiles from schools and hospitals, asserting that the United States will not become trapped in an endless war. The Pentagon has confirmed that 140 US soldiers were wounded during the 10-day Operation Epic Fury, with eight personnel in critical condition and seven reported killed.

Hegseth has indicated that President Donald Trump will determine the terms of Iran’s surrender amid investigations into attacks on schools in southern Iran. The Defence Secretary has suggested that war against Iran could last up to 8 weeks, with the United States claiming control of airspace and having disabled Iranian missiles.
