BPJS Health Insurance Contribution Rates as of 13 March 2026
Jakarta — Indonesia’s Health Minister Budi Gunadi Sadikin stated that official increases to BPJS health insurance contributions will only affect upper-middle-class members who pay contributions independently, such as those currently paying approximately Rp 42,000 per month.
The increases will not impact low-income groups. The Health Minister noted that participants from deciles 1 to 5 remain subsidised by the government through the Contribution Assistance Recipient (PBI) scheme.
“If tariffs are increased for poor people in deciles 1-5, there is no effect. Because the poor are paid for by the government,” said Budi Gunadi Sadikin, commonly known as BGS, on Friday 13 March 2026.
He emphasised that the BPJS system adheres to the principle of social insurance with a cross-subsidy scheme. This means participants with higher economic capacity help support the financing of less affluent participants.
“The concept of BPJS social insurance is that wealthy people subsidise the poor. It is like taxation. Taxes mean wealthy people pay more, but access to roads is the same,” he said.
According to Presidential Regulation 63/2022, the contribution calculation scheme for participants is divided into several categories:
First, participants in the Health Assurance Contribution Assistance Recipient (PBI) scheme have their contributions paid directly by the Government.
Second, contributions for Wage-Earning Workers (PPU) employed in Government Institutions, comprising Civil Servants, TNI members, Police members, state officials, and non-civil servant government employees, amount to 5% of monthly salary or wages, with 4% paid by the employer and 1% by the participant.
Third, contributions for PPU employees working in state-owned enterprises (BUMN), locally-owned enterprises (BUMD), and private companies amount to 5% of monthly salary or wages, with 4% paid by the employer and 1% by the participant.
Fourth, contributions for additional family members of PPU, including the fourth child onwards, parents, and in-laws, amount to 1% of salary or wages per person per month, paid by the wage-earning worker.
Fifth, contributions for other relatives of PPU such as siblings or in-laws, domestic assistants, and others, non-wage-earning worker participants (PBPU), and non-worker participant contributions have their own calculations:
Rp 42,000 per person per month with benefits for Class III ward services. For Class III specifically, from July to December 2020, participants paid Rp 25,500, with the remaining Rp 16,500 paid by the government as contribution assistance. From 1 January 2021, Class III participants pay Rp 35,000, while the government continues to provide contribution assistance of Rp 7,000.
Rp 100,000 per person per month with benefits for Class II ward services.
Rp 150,000 per person per month with benefits for Class I ward services.
Sixth, health insurance contributions for Veterans, Independence Pioneers, and widows, widowers, or orphans of Veterans or Independence Pioneers, are set at 5% of 45% of the basic salary of Civil Servants in grade III/a with 14 years of service per month, paid by the Government.
Under the contribution scheme stipulated in Presidential Regulation 63/2022, payment must be made by the 10th of each month. There is no late payment penalty as of 1 July 2016. Penalties are imposed if, within 45 days of when membership status is reactivated, the participant receives inpatient health services.
Based on Presidential Regulation 64/2020, the amount of service penalties is 5% of the initial diagnostic cost of inpatient health services multiplied by the number of months in arrears, with the following provisions:
The number of months in arrears is at most 12 months.
The maximum penalty amount is Rp 30,000,000.
For PPU participants, payment of service penalties is borne by the employer.
