The Indonesian Army has formed an investigation team following an explosion at the Central Munitions Depot II belonging to the Army Equipment Centre in Madiun, East Java, on Thursday (16/7).

Head of the Army Information Service, Brigadier General Donny Pramono, stated that the team would work professionally, objectively, and comprehensively to ensure every fact obtained is accountable.

"We have formed an investigation team to conduct a thorough inquiry and investigation to ascertain the exact chronology and cause of the incident," Donny said during a press conference on Thursday.

He explained that, based on initial reports, the incident occurred while personnel were carrying out inspection and maintenance procedures for munitions materiel in one of the storage warehouses.

In the incident, one Army personnel member was confirmed dead, four others sustained serious injuries, and two more suffered minor injuries.

"Immediately after the incident, the unit evacuated all victims to hospital for rapid medical treatment while reporting the matter procedurally through the chain of command," he said.

He added that the Army has taken initial handling steps by providing full attention and assistance to the victims.

"The investigation team has been formed and departed for the scene this afternoon. The Army is clearly committed to handling this incident seriously and transparently, whilst ensuring every step in caring for the victims and the investigation process proceeds as effectively as possible," he stated.
