A recent clinical study conducted by Pennsylvania State College of Medicine indicates that alternative tobacco products have the potential to increase the chances of adult smokers moving away from the habit while reducing exposure to harmful substances produced by tobacco combustion. The research examined the use of several alternative tobacco products, including electronic cigarettes, heated tobacco products, and nicotine pouches. The results show these products can serve as a harm reduction approach for adult smokers who have not succeeded in quitting through other methods.

Associate Professor of the Department of Public Health Sciences at Penn State College of Medicine, Jessica M. Yingst, stated that the findings provide an important perspective in efforts to help adult smokers reduce health risks associated with smoking. “For smokers who have not been able to quit smoking through recommended approaches, this study shows that switching to alternative tobacco products is associated with a real reduction in exposure to harmful toxicants while supporting the transition away from smoking. This is a meaningful finding for public health,” Yingst said.

The study, titled “Toxicant Exposures After Switching From Cigarettes to a Pod-Based Electronic Cigarette” and published in May 2026, showed that participants who used alternative tobacco products in the short term were approximately three times more likely to switch from cigarettes compared to those using non-nicotine products. Researchers also found a decrease in several biomarkers of exposure to harmful substances in the group using alternative tobacco products. This finding highlights the role of the tobacco combustion process as the main source of various harmful substances linked to smoking.

According to Yingst, nicotine is indeed addictive, but it is not the primary factor causing the diseases commonly associated with smoking. The risk mainly comes from harmful compounds formed when tobacco undergoes combustion. “Adequate nicotine delivery is a key factor in the switching process. Alternative tobacco products provide nicotine to consumers while significantly reducing exposure to various harmful chemicals,” she explained.

Paido Siahaan, Chairman of the Indonesian Vape Consumer Association (Akvindo), assessed that the research results further strengthen the evidence regarding the potential of alternative tobacco products to help adult smokers reduce exposure to harmful substances when switching from cigarettes. According to Paido, study participants still obtained nicotine, but with a lower level of exposure to harmful substances. He believes this reinforces the understanding that the tobacco combustion process is one of the main sources of harmful substances. “This study shows that participants still obtained nicotine, but with reduced exposure to harmful substances. This finding further strengthens the understanding that most harmful substances come from the tobacco combustion process, so by eliminating the combustion process, exposure to a number of harmful substances can be reduced,” Paido said.

Nevertheless, Paido cautioned that alternative tobacco products are not risk-free. These products are intended for adult smokers who want to switch from smoking, so public education and distribution monitoring still need to be strengthened. “Education about alternative tobacco products needs to be conducted regularly so that more people understand that these products are not for minors, pregnant women, or non-smokers. For adult smokers who have not been able to switch from smoking, using alternative tobacco products that do not involve combustion can be a more rational harm reduction approach compared to continuing to smoke,” Paido concluded.
