Rp 20 Trillion Injection for BPJS Kesehatan Stalled by Regulation
Health Minister Budi Gunadi Sadikin has stated that the government cannot yet disburse a Rp 20 trillion fund injection to the Social Security Administering Body (BPJS) for Health, despite having received approval from the Ministry of Finance. The disbursement of the funds remains constrained by regulatory issues.
"There is a problem from the regulatory side, regarding how the disbursement is made," Budi said after attending an event at the Grand Sahid Jaya Hotel in Jakarta on Friday, 10 July 2026.
Budi explained that the disbursement of the funds requires a Government Regulation (PP) on Asset and Liability Management (ALMA). This rule will serve as the legal basis for the government to channel bailout funds to BPJS Kesehatan. The draft of the Government Regulation, he said, has been submitted to the Ministry of the State Secretariat and is currently still under discussion.
Budi expressed hope that the regulation could be signed soon by President Prabowo Subianto, so that the Ministry of Finance can immediately disburse the funds that have been prepared. "Hopefully it can be signed soon by the President, thus enabling the Ministry of Finance to disburse it as quickly as possible. We expect it to be completed within a month or two," he said.
Previously, BPJS Kesehatan President Director Prihati Pujowaskito conveyed that the agency’s financial condition is currently in deficit. This is reflected in the claims ratio, which has reached 108 percent. Each month, BPJS Kesehatan spends around Rp 16 trillion to pay hospital claims, while premium revenues only amount to around Rp 14 trillion.
As a result, BPJS Kesehatan experiences a deficit of approximately Rp 2 trillion every month. This figure is estimated to swell to around Rp 30 trillion by the end of 2026. Nevertheless, Prihati stressed that this condition is not a new issue. According to her, BPJS Kesehatan has faced a similar situation since 2018–2019. "This deficit is not caused by the current board of directors’ leadership. Therefore, BPJS’s main focus is maintaining sustainability through strengthening risk pooling, robust funding, quality and cost control, and improving service quality for participants," Prihati said.
Prihati stated that BPJS Kesehatan is targeted to receive the Rp 20 trillion fund injection by August 2026 at the latest. Without this additional funding, BPJS Kesehatan is estimated to only be able to meet its claim payment obligations until July 2027 before facing the risk of default.
