Public Communication for Free Health Screening Programme: Massive but Uneven

The Free Health Screening (CKG) programme, launched in February 2025, is touted by the government as the largest health initiative in Indonesian history, targeting all citizens from newborns to the elderly. As the Ministry of Health shifts its focus in 2026 from screening coverage to the handling of results, officials have acknowledged that strengthening public communication is essential to ensure the public understands and follows up on their diagnoses. However, research indicates that the programme’s communication strategy, which relies heavily on mass and social media, is more effective at reaching middle- and upper-income groups, while lower-income populations remain underserved.

Studies conducted at community health centres and a systematic review of chronic disease management programmes reveal that participation is low among the elderly, rural communities, and the poor, not due to rejection, but because the communication channels do not align with their needs. These groups respond better to direct, interpersonal, and community-based approaches rather than digital literacy campaigns. The findings dismantle the common assumption that massive socialisation automatically equates to equitable outreach.

The risk for the CKG is not total failure, but partial success: reaching those already easy to access while leaving behind the most vulnerable. Experts argue that public communication must be treated as a core component of the programme’s success, not an administrative afterthought. Without segmenting messages by target group, evaluating participation data by region and socio-economic class, and strengthening grassroots communication pathways, the programme may achieve national policy coverage but see its impact concentrated among those already better served.
