Maintaining adequate vitamin B12 levels according to current health guidelines may not be enough to protect aging brains. Researchers at the University of California, San Francisco (UCSF) have identified a group of elderly individuals with biologically active B12 at the lower end of the ‘normal’ range who nonetheless show signs of slowed cognitive processing, difficulties with visual processing, and white matter damage in the brain.

Vitamin B12 is known to play a crucial role in DNA synthesis, red blood cell formation, and nerve tissue health. However, the study, published in the Annals of Neurology, suggests that the current minimum thresholds may miss early signs of nervous system dysfunction.

The study involved 231 healthy participants with an average age of 71 who did not have dementia or mild cognitive impairment. The average blood B12 level was 414.8 pmol/L, well above the US minimum threshold of 148 pmol/L. Unlike standard tests, the researchers focused on the biologically active form of B12 to determine how much of the vitamin could actually be utilised by the body.

After adjusting for age, sex, education and cardiovascular risk, the team found that participants with low active B12 had slower cognitive processing speeds. The effect strengthened with age.

MRI scans supported the warning. Participants with lower active B12 levels had higher white matter lesion volumes—brain injury areas often linked with increased risk of cognitive decline, dementia and stroke.

“Earlier studies that defined a healthy B12 level may have overlooked subtle functional manifestations of high or low levels that affect people without obvious symptoms,” said Dr. Ari J. Green of the UCSF Department of Neurology and the Weill Institute for Neurosciences. “Reconsidering the definition of B12 deficiency to include functional biomarkers could lead to earlier interventions and the prevention of cognitive decline.”

As people age, the body’s efficiency in absorbing vitamin B12 tends to decline. Other factors such as use of certain medications, digestive disorders, and low intake of animal products can further raise the risk of reduced B12 levels.

Beaudry-Richard, MSc, one of the lead authors, stated that a low B12 level that is technically still considered normal could have a broader impact than previously thought.

“Beyond redefining B12 deficiency, clinicians should consider prescribing supplements to older patients with neurological symptoms even if their B12 levels are within the normal range,” Beaudry-Richard said. “Ultimately, we need to invest in more research into the biology underlying B12 inadequacy, as it may be a preventable cause of cognitive decline.”

The study sends an important message to the medical community and society: lab results indicating B12 within the “normal” range may not guarantee optimal brain health, particularly if subtle changes in memory, processing speed or visual acuity are already emerging.
