Using hair transplants for male baldness<\/p>\n
JAKARTA (JP): What a shock to look in the mirror and see the
\narea of thin hair on the top of your head is widening and getting
\nthinner! What can you do to stop the process?<\/p>\n
Surveys show that 1.5 percent of men experience male pattern
\nbaldness (MPB). They are men who inherit the trait from their
\nparents and the hair loss occurs when they have reached the age
\nof 40 years or older.<\/p>\n
\"It is proven that the male sex hormone, in this case
\nandrogen, plays a role in male baldness,\" said a dermatologist
\nand veneorologist at Persahabatan Hospital, Adityawarman, during
\na discussion held by the study group of the Indonesian Skin
\nSurgery and the Jakarta Skin Center last week.<\/p>\n
However, he pointed out that androgen-related baldness was not
\nstrictly a male problem, and could also occur in women with high
\nlevels of male sex hormones, a condition called adrenal virilism.<\/p>\n
Adityawarman explained that this type of baldness only caused
\nhair loss in specific areas on the scalp, which include the
\nfrontal area (forehead), occipital (the back part of the head),
\ntemporal (the sides of the skull) and the vertex (top of the
\nhead).<\/p>\n
\"Doctors still cannot explain why baldness occurs only in
\nthose particular areas of the scalp,\" he said.<\/p>\n
To remedy the condition, Adityawarman said a hair transplant,
\na procedure that requires taking hair grafts from other parts of
\nthe scalp to be transplanted to the affected areas, could be
\nperformed to cover the bald spots.<\/p>\n
\"With this procedure, the hair will grow the way it did in its
\noriginal areas (donor areas); its growth is not determined by the
\nlocation where it is implanted,\" Adityawarman said.<\/p>\n
Patients suitable for this procedure are those between the
\nages of 20 and 40 who are in good health.<\/p>\n
People with diabetes mellitus, hypertension, diatesis
\nbleeding, drug allergies, cardiovascular problems or scalp
\nproblems such as dermatitis seboroic and recurrent foliculitic
\nare not eligible for the procedure.<\/p>\n
Adityawarman underlined the importance of giving patients
\nadequate information about the procedure, including the
\ntechniques employed, duration and possible complications during
\nand after the procedure.<\/p>\n
Patients should be informed in advance about possible
\ncomplications like bleeding, infection, edema (accumulation of
\nfluid in cavity or connective tissue) and pigment irregularity.<\/p>\n
There are also standard esthetics that should be followed by
\ndoctors to make sure that the results of the procedure look
\nnatural.<\/p>\n
\"Patients may be unaware that the human face is naturally
\ndivided into three parts, each accounting for one-third of the
\ntotal face. The first third is the span between the hairline and
\nthe glabella area, the second is between the glabella and
\nsubnasale and the remaining is between the subnasale and menton.<\/p>\n
\"We will explain this to the patients so they will not ask for
\nthe transplant to cover wider areas than the proportional
\nstandard. That would look unnatural,\" he said.<\/p>\n
Doctors will likely turn away patients who have mental
\nproblems, are depressed and those with unrealistic demands, he
\nsaid.<\/p>\n
As for the procedure itself, Adityawarman mentioned five
\nconsiderations before the doctor performs the transplant. These
\nconsiderations include hair color, because gray hair is hardly
\nvisible; type of hair, as curly hair is more difficult to
\ntransplant; and hair thickness, because thick hair is preferred
\nfor transplanting.<\/p>\n
The procedure begins with the administering of anesthetics to
\nboth the donor and recipient areas of the scalp. The doctor will
\nthen perform either a standard graft, which requires four-
\nmillimeter-deep punches to the donor area; a minigraft, using a
\nmultiple blade knife to take three to eight hairs; or a
\nmicrograft to take one to two hairs. The graft is then stored on
\na hygienic plate soaked with saline solution. The punched gaps on
\nthe donor areas are closed with a glycolate absorbable thread or
\npolydiaxone subcuticular and prolene.<\/p>\n
The grafted hairs are then cleansed, with foreign particles
\ncarefully removed without damaging the delicate layer of natural
\noil.<\/p>\n
The doctor will then make slits or holes where these hairs are
\nto be implanted. The hair fragments are then inserted according
\nto the direction of hair growth.<\/p>\n
\"A combination of punch graft and minigraft\/micrograft will
\ngive a natural look,\" he said.<\/p>\n
After the hair implant, the scalp will be covered with a kling
\nbandage for the next 12 to 18 hours to secure the grafted hairs
\nin place.<\/p>\n
\"After day three or four, the graft locations will start to
\nheal and on day 10 to 14, the stitches in the donor areas can be
\nremoved,\" he said.<\/p>\n
Transplanted hairs will shed within several months but will
\nregrow in the third month at a pace of three to five millimeters
\na month.<\/p>\n
\"The end result will show 12 to 18 months after the
\nprocedure,\" he said.<\/p>\n
There are cases where the grafted areas end up with a
\nperforated finish after recovery. \"This can be overcome by
\ndermabrasion or electrodesiccation to smoothen the perforated
