Dr Santi Christiani Gultom, a Specialist in Internal Medicine with a\nsubspecialty in Haematology and Medical Oncology at MRCCC Siloam\nSemanggi, explains that breast cancer therapy can differ between one\npatient and another, even if they share the same type of cancer.<\/p>\n
Santi stated that doctors do not determine therapy solely based on\nthe organ where the cancer is found, but also consider a number of\ndisease characteristics and the patient\u2019s overall condition.<\/p>\n
\u201cIn reality, we do not treat based on the cancer alone, but based on\nthe organ,\u201d Santi said during the MRCCC Siloam Semanggi Media Hospital\nTour 2026 in Jakarta on Tuesday.<\/p>\n
She explained that the stage and the presence of metastasis are key\nfactors in determining therapy options. Patients with early-stage cancer\nmay receive a different treatment approach compared to patients whose\ncancer has already spread.<\/p>\n
Doctors also assess the aggressiveness of the tumour through\nmolecular profiling, including the status of estrogen receptor (ER),\nprogesterone receptor (US), human epidermal growth factor receptor 2\n(HER2), and Ki-67.<\/p>\n
According to Santi, these molecular characteristics help doctors\nidentify breast cancer subtypes that require different treatment\napproaches, such as Luminal A, Luminal B, triple-negative, and\nHER2-positive.<\/p>\n
\u201cThere are several types of breast cancer, from Luminal B and Luminal\nA to Triple Negative or HER2-positive, and each has different\ntreatments,\u201d she noted.<\/p>\n
In addition to cancer characteristics, doctors consider each\npatient\u2019s individual condition, including age, physical condition, organ\nfunction, and comorbidities such as heart disease, kidney disorders, and\ndiabetes.<\/p>\n
\u201cWe cannot administer cancer medication arbitrarily; it must be\ntailored to each patient and their comorbidities,\u201d said Santi.<\/p>\n
She added that the objective of the treatment also determines the\ntherapeutic approach. Patients undergoing curative-intent therapy may\nreceive more aggressive treatment, whereas palliative therapy for\ncertain patients aims to extend life and maintain the patient\u2019s quality\nof life.<\/p>\n
